As was the case in 2021, our insurance policy is requiring a waiver as an addendum for every participant in any KCDG event in 2021.  Event and league directors will be checking a list of participants who have signed their waiver.  If you do not sign the waiver, you cannot compete on any event on a KCDG course.  Signing this waiver will apply to all events in the 2021 season, so you’re only required to sign it once.  If you signed the waiver in 2020, you’ll need to sign a new waiver for 2021.  Follow the instructions below, get your waiver signed, and we’ll see you on the course!

Navigate to https://kcdiscgolf.org/waiver-form/.  You can do this on a mobile device or on your desktop/laptop computer.

Fill out all the fields on this form and click submit. It will redirect you to the e-signature form with all the data you just filled out populated. NOTE: This may take a moment as the waiver is being generated.

Read through the waiver, and initial by each paragraph. If you are under the age of 18, your parent’s information is required as well. Click on the signature box. You can either draw your signature on your smart device, draw with a mouse, or sign with text.

Once everything is completed, click the Agree & Sign Button. NOTE: This may take a moment as the waiver is being processed.

Once this successfully processes, you’ll be taken to a screen that shows it was a success. The waiver will appear as a PDF in your email as well as stored by KCDG.

And now you’re good for the entire 2021 KCDG season!

If you are taken to the waiver screen (the 2nd screenshot above) and don’t sign, a link will be emailed to you to complete the waiver.
