2024 KCDG Event & League Waiver

Please read through the following information and sign the form below:

I acknowledge and fully understand that my participation, (or that of my minor or ward if under 18 years of age), in the Kansas City Disc Golf KCDG sponsored events may involve the risk of injury, including the possibility of serious injury or death, not only from my own actions, but also from the action or inaction of others, (whether involved in the event or not), the condition of the premises, or the failure of the equipment used.

I agree to conduct myself during the KCDG sponsored event with all due regard for my own safety and the safety of all participants, observers and bystanders. I agree to comply with the directions of all KCDG sponsored event organizers, and I agree to immediately notify KCDG sponsored event organizers if I observe any unsafe condition or behavior during the KCDG sponsored event.

I, and on behalf of my heirs, assigns, executors, personal representatives and next of kin (and/or on behalf of my minor or ward and their heirs, assigns, executors, personal representatives and next of kin), hereby release and forever discharge KCDG and its agents from all claims, demands, liabilities, actions, and causes of action, what so ever, arising out of or relating to any injury, damage, or loss, including claims due to the KCDG’s own negligence, failure to protect or warn, and transmission of COVID-19 or other communicable disease, which I may sustain in any way connected with my person or property, relating to my attendance or participation at any KCDG league, tournament or other sponsored event, and I voluntary assume any risks of injury or damage to my property by my participation at said event.

I have read this waiver and release agreement, fully understand its terms, and understand that I have given up substantial rights by signing it, and I sign it freely and voluntarily without any inducements. Furthermore, I also recognize that the KCDG and the KCDG sponsored event organizers have the right and responsibility to record the event through video and still photography of any and all participants. I hereby grant permission that such photography may be used for event publicity, public information, or any other promotional use that falls within KCDG and the KCDG sponsored event organizers activity.
